How to Create a Personal Trainer Appointment

Physiotherapists or other ACC providers may need to be able to make Personal Trainer appointments (aka. PT Appointments).
This is useful. for example, if ACC registered providers have appointment types unrelated to ACC billing.
In this tutorial, you will learn how to change a clinician's settings to allow to create PT appointments.
Prerequisites
The clinician/provider has been created: see How to Create a New Clinician
The system user should have the access permissions to modify the clinician's settings
The Creditor has had a bank account created against it.
